Position Summary:
This position is responsible for the design, implementation, and delivery of clinical and therapeutic services across Thrive Group Community Services locations (Community Services). Provides direct counselling, intake assessments, family assessments, and advocacy with diverse populations in accordance with client care plans. This position includes supporting clients with a variety of physical and psycho-social conditions living in the community and addressing social determinants of health, mental health concerns, and system navigation challenges. This position includes delivering client, caregiver and employee education initiatives. The position provides clinical and therapeutic leadership to the Thrive Group Community Services teams and programs and ensures the implementation of best practice approaches, fostering a culture of interdisciplinary collaboration, and cooperation. Additionally, the position works in collaboration with the Community Services senior leadership team to ensure streamlining and coordinating of how therapeutic and clinical services are designed, implemented and delivered.
Employment Guide:
- A Master of Social work or related discipline, along with 2-3 years of practice in a social work role or a combination of education and experience are requirements for this position.
- Current registration in good standing with the OCSW&SSW without terms, limits or conditions.
- An asset to be registered with the Ontario Association of Social Workers.
- Demonstrated knowledge of psychosocial theories, family dynamics, conflict resolution and mediation, aging processes, behaviour management techniques and system barriers
- Must demonstrate approachable, and professional demeanour and have a sound understanding and empathy for the needs of diverse populations in the community
- Strong interpersonal skills are essential along with ability to integrate good human relationship skills and professional standards into client-centric practice.
- Must demonstrate good written and oral communication skills and ability to work in collaboration within an interdisciplinary team as well as work independently
- Familiarity with Microsoft Soft 365 programs, Alayacare, and RAI-MDS is an asset.
- A valid driver’s license is required as well as successful completion of a vulnerable sector police check.
Job Type: Full-time, permanent. 40 hours per week.
